We had this. The anti-backlash nuts were binding. We took them apart, chased the threads with a tap and re-assembled with slightly less pre-load. Hasn't been a problem since.

I dealt with the same issue for a month or so. We jumped through a lot of hoops, replaced both lead screws, verified the motor was good, a ton of stuff. Ended up being an intermittent wire connection between the control board and the conveyor width
