What kind of vision system does your machine have? If it is AIS3500AX, that can be the cause. AC supply is hardly the reason...

This looks like faulty VGA card (the one from which VGA cables come out) in the VME rack. They are notorius and fail often. Sometimes you can replace VRAM chips and fix it...
