Could you please share me what are the possible reasons for the DC DC Converter failures. Application of the subjected DC DC Converter is to separate the power & control ground. We are using DC DC Converter in regulator O/P, even though converter is failing. Is regulator can not suppress the spikes?

you are not clear on the nature of your failure. What does "failure" mean? There are a 1000 different reasons for your "failure" but if you specify, maybe we can knock it down to a dozen or so.
