Just a basic suggestion.... Inspect the nozzle placing the component...because that is the only medium, apart from the PCB design, which can be causing the damage. It occurred in my Department.

Use multiple small stencil openings [15 thou square], where total area approximately 50% of total PCB ground pad. What's the problem?

I agree with the stand alone option. That way you can sample PCB's from different stages ( print, placement post reflow) In a controled process, after reflow inspection would be ideal.

Assembling boards with 12 pitch are very difficult to control. We like flat surfaces on boards with components below 25 pitch.

You may to reduce a little bit more with HASL. for .4mm pitch and such I would maybe go to a 5 mil with your standards.
