We had a similar problem once, it was due to insufficient v-score depth. While waiting for remade PCBs we would run an exacto knife along the score lines prior to depanel to cut a little deeper. Not ideal for sure, and not fast, but it worked.

Deepening the V-scores was in vain. No change. This board is on Rev R now, with no changes to material/array/v-score spec, and this is the first we've seen this problem. I just don't get it.

We have the same ovens, our 2 sided boards have the same profile for both sides. Haven't seen a problem in 8 years.

CHECK IO BOARDS STATUS, CHECK HEAD IO 24VDC SUPPLY, CHECK 24VDC SUPPLY TERMINAL, CHECK WIRING CONNECTED TO HEAD IO I think this is an I/O communication problem. Fail in connecting to I/O cards.

We have an older Checksum with intermittent bootup problems. Factory does not support, looking for someone with knowledge or might know someone who supports these older systems. Thanks

@JimmyB You have right, I'm not from Chicago - I didn't go to overseas yet. @All Thanks guys for your help but I haven't the right password. I think I need to remove and reinstall the OIS. That's the only one solution to solve my problem.

Thank you SteveO, I have also tried reflow 2nd time without any problem, but I also do not know the impact of long term reliability as our devices are most likely operated in the harsh environment. We are currently do soldering in Argon environment.

We had this. The anti-backlash nuts were binding. We took them apart, chased the threads with a tap and re-assembled with slightly less pre-load. Hasn't been a problem since.
