I agree with DucHoang. If your board and reflow profile have not changed then I would suspect incoming parts. We've seen tombstones when a chip supplier had plating problems.

Thanks for the input Pete. I am using CP4 8x2 feeders. I have tried first with 0.7mm nozzle. Although pick up was OK, the camera saw a pregnant 0402 part which is as you know is 1mm by 0.5mm. I then moved to specially made 0.6mm nozzle to minimise th
