We used to use Titanium inserts when there wasn't much clearance between SMD and thru-hole leads. I don't recall how tight we could get though.
Printed circuit boards lacked with a green solder mask are - including SMT components - essential for soldering safely. Learn more: http://www.lpkf.com/products/rapid-pcb-prototyping/solder-masks-legend-printing/index.htm?utm_source=youtube&utm_mediu

vehicles.Components were mechanically redesigned to have small metal tabs or end caps that could be directly soldered to the surface of the PCB. Components became much smaller and component placement on both sides of a board became far more common with surface mounting than through-hole mounting, allowing much higher circuit densities and

– Layer assignment is critical for designing a printed circuit board that will function. Pads for SMD components must appear on the surface layer, though-hole components must have defined layer spans for holes, and silkscreen and mechanical outlines need to be defined in their own layers
