We used to use Titanium inserts when there wasn't much clearance between SMD and thru-hole leads. I don't recall how tight we could get though.

The opening in the wave pallet for the TH connection in question would be a titanium insert rather than just a cutout in the durastone like normal. They're able to hold tighter tolerances with the titanium vs durastone as far as I understand.

Why don't you use peelable/washable solder mask on the SMT components next to the T/H pins? Then, you won't have to worry about keeping the tight tolerances.

Titanium wall could go down to 10mil thickness. Just make sure this location runs on the back end of the pallet flow to avoid shadowing and allow solder to wet in this area.

What is your current paste coverage? You mentioned windowpane and you mentioned 5mil stencil. You did not mention if PAD design meets recommendation? If it does, go 6mil and more paste coverage to increase volume.

OSP finish is the big issue here. You almost can not get 100% coverage solder on OSP with 1 over 1 aperture stencil. Reconsider solder coverage requirement in this case. Check your peak temp in profile may help a little.
