We have seen this problem several times. It always goes back to pad solderability, very seldom does it have anything to do with ball size. If there is no evidence of wetting on that pad, but all of the other pads did solder, you need to ask yourself

John, I believe that your problem is cuased by inadequate filling of the transformers with potting material - if any is used at all. When you attempt to clean, particularly with a chemistry, it is designed with wetting agent, surface tension modifier

Dhanashekar, Yes this is an important proces control tool. This will show if you printer setup is correct however, I put more importance on coverage. If your paste height is 1 mil shorter than it should be, you may need to look at your printer setup
