I think it is the belt. Looking at the video it appears to come off just after the splice goes around the pulley, so probably a bad splice. Jerry

Thank you guys for all of the feedback. I ended up swapping the left and right conveyor belts and they seem to have been working properly for 24 hours.

Yes something like that.ie a higher edge. Looking at your original picture I assumed no edge. If that doesn't work, then it is time to change the belt. But do check the frame with a straight edge. you may find a little twist or bend that may not be v
