It didn't see the ref points on the bottom of the hydra head. start by checking the camera for dirt or dropped parts. Is it a hydra camera or Linescan?

Placement numbers are 4 digits long, the first digit is the head# followed by a 3 digit placement number. For example if you adding a local fiducial for placement #3 you need to enter 1003.

Why was it down before? It's possible you have bad cabling. Voltage will read okay but it/they will require very little resistance to work properly (like less than 1.5 ohms).
