I have this problem in FR4 large double sided pcb's. My soldering line is SOLTEC If you're talking about outgassing from plated through holes, moisture and/or entrapped chemical residue is most often the problem. If the boards have been properly processed and meet all other specified quality requirements, a pre bake cycle is recommended to eliminate entrapped moisture. A pre bake cycle of 3 hours at 250 F., within 24 hours (depending on climatic conditions as relative hunidity, etc.) will do the job. If other than moisture, you have more serious problems that cannot be rectified short of ensuring your supplier effectively manages processes.
