The best verification system combined with bad operator equals POOR RESULT. I think the real question is how to find a good operator and once you have him/her how to keep them? I am in process of losing one of my best operators(no challenge).

Evtimov, I agree. I'm leaning more towards the quality of worker rather than software, etc. Since this post this incident happened AGAIN! This time there was ZERO verification performed at start up.

What pick and place machine are you using? We use Mydata's with a software package for easy loading and traceabillity (Mylabel). We also have electrical verification checks on the first 3 picks of any feeder to ensure they are the correct parts.

Are you using specific tooling for each part, or just generic? If so it could be pushing down at an angle and slightly tearing the PCB. We use the same style press as you but make custom fixtures to fit each connector and hold each PCB tight.

Well both materials are thermo plastics and recyclable. But it is diffficult to separe into streams. Do you have any process to do this?

But do you separate this waste from others. Or do you separate by plastic? Because if you do the second it would be easier to find a way to recycle. Do you use an optic sorter ?

Are you able to turn off mechanical centering? Or can you adjust the settings?

The only reason you would need to run this calibration is if the camera was replaced or the centering camera got whacked which is highly unlikely. If your placements are off, put an offset in the parameter file.
