I've seen problems where the component was slightly thicker than the tape and popped out of the pocket upon transport. Is the component still in the pocket after mispick? Do you have access to a high speed camera or could rent one for a day?

Kris, Get that high speed camera in there. You have to find out what is happening after the pick point. Also just to throw in another possibility into your cause list. How about static electricty? It can effect such small components. My $ .02

Have you tried using offsets to begin to move the head away in the Z axis while it is moving in a lateral ( X Y ) direction. The spikes are difficult on the Ersa, but experiment with hitting the location from different directions may help also.
