I agree with James. Granted there are a variety of lasers to try, we too could not get a consistent power setting. Plus when paste turns liquid, you get some reflection. Also the mask around the pad has to be pulled back or burning may occur.

If the vias are large and don't always tent, you can have your pcb supplier double mask the pcb. I do this for COB assembly sometimes because the globtop wicks through the vias and also causes air bubbles in the globtop.

Depends on your board? What finish? Solder mask between all pads? What pitch is the part? Will it machine placed? Don't forget proper profiling as well. 1:1 with machine placement should yield no issues.

Poor training. Don't blame the operator, he was either shown this or left to his own devices. Be the better man and provide him a better process. Regular old peelabale solder mask would be a start.
